Dear Dr:

The patient was visited and the medical hx was reviewed, including medication and laboratory data. The patient with mental illness was admitted to TTPC, under regular treatment and follow-up. Please beware of side effect of medication and keep management as your experience.



Problems:
--Hyperglycemia, DM
--HTN, HCVD
--Hyperlipidemia

本院有藥物可自行開立
但若外院有定期替病人作檢查，可續外診以利安排

本院至少3個月抽一次肝功能
外院至少半年一次腹部超音波

Suggestion:
--Diet control(may consult dietitian for suggestion)
--Activity control
--Please keep regular f/u as your routine
--Check HbA1c/AC, CHOL/TG/LDL/HDL, GOT/GPT, Cr, Uric acid every 3 months(可視檢驗結果調整項目)
--Follow electrolyte/CBC regularly
--Please follow thyroid function and thyroid echo every 6 months(because of medication for mental disorder)
